You’ve warmed up, you’ve drunk your water, and it keeps happening. If that sounds familiar, there’s one cause worth knowing about before you buy another throat spray: silent reflux.

This guide is for working and aspiring voice actors. It covers what silent reflux is, why people who talk for a living meet it so often, the clues you can hear in your own recordings, the everyday habits that clinics suggest, and the signs that mean it’s time to see a doctor. It isn’t medical advice, and it won’t diagnose you. It will help you ask better questions when you sit in front of one.

What “silent” actually means

Most people know reflux as heartburn, the burning feeling behind the breastbone after a big meal. Silent reflux is the same idea with a different route. SingHealth, the Singapore hospital group, explains on its patient page: “Laryngopharyngeal reflux (LPR) occurs when stomach contents backflow or reflux via the oesophagus into the throat (pharynx) and voice box (larynx).” It goes on to say that “Unlike gastro-oesophageal reflux disease (GERD), it often does not cause heartburn, earning it the name ‘silent reflux’.”

That’s the catch. You can have it without ever feeling a burn in your chest. The same page offers a reason: the oesophagus “is physiologically better equipped to resist mucosal damage and irritation from stomach contents as compared to the larynx and pharynx.” In plain words, the tube that carries food is tougher than the delicate tissue around your vocal folds. A tiny amount of stomach contents that the oesophagus shrugs off can still irritate your voice box.

The US National Institute on Deafness and Other Communication Disorders lists “inflammation caused by gastroesophageal reflux” among the causes of voice problems, next to upper respiratory infections and vocal overuse. So it’s a recognised cause, not an internet theory.

Why working voices tend to meet it

Voice work has a few habits that don’t help. Long reads in a quiet room, often sitting in a slightly hunched position. Sessions scheduled around the client’s day rather than your digestion. A snack and a coffee between auditions, then a big dinner late because the last job ran over. None of this is a character flaw. It’s just how a freelance day tends to go.

There’s some research behind the worry. A 2025 abstract from the Voice Foundation, by Brittany Hathway, Jeremy Best, Robert Weber, Lauren Holland and Bradford Perrine, looked at college vocal performers. The team starts from this point: “Performers may be at increased risk of reflux due to the physiology of singing and lifestyle factors.” It covered 256 students from four performance majors who had voice screenings, and about one in five of those who had a throat examination were diagnosed with laryngopharyngeal reflux. The authors put the prevalence of “reflux-related laryngeal changes in college vocal performers” at “around 20%.”

These were singers, not voice actors, and it was a single university clinic, so we shouldn’t stretch the number too far. But a voice actor uses the same instrument for more hours than most singers do, and the logic carries across.

“These data suggest the treatments they are using do not adequately resolve laryngeal changes related to reflux.”

Hathway and colleagues, Voice Foundation abstract, 2025

That finding is worth sitting with. Eleven of the students were being treated for reflux, and the treated group was just as likely to show reflux changes in the throat as the untreated group. It doesn’t mean treatment is pointless. It means that taking one tablet and carrying on exactly as before may not be enough, and that’s a good reason to see a specialist rather than self-prescribe.

© The Voice Realm

What it sounds like in your recordings

You’ll often hear it before you feel it. SingHealth lists these symptoms: “persistent throat discomfort, globus sensation (feeling of a lump in the throat), hoarseness, chronic cough and excessive throat clearing.” The longer list on the same page adds throat itchiness and excess phlegm.

For a voice actor, those show up as practical problems:

A voice that sounds fine at ten and gravelly by two, with the roughness worse after a meal or first thing in the morning.

A need to clear your throat between lines, which leaves a harsh little scrape in the file and irritates the folds further.

Extra wet clicks and swallowing noises on the recording. If you’ve already worked through how to stop mouth clicks and they keep coming back, it’s worth asking whether the cause sits lower down.

Coughing fits that arrive on cue, usually just after you lie down or after a heavy meal.

Many of these have other causes, from allergies to a cold to plain tiredness, so treat the list as a reason to pay attention and not as a verdict. The NIDCD’s own checklist is simple: it asks whether your voice has become hoarse or raspy, whether your throat often feels raw, achy or strained, and whether you find yourself “repeatedly clearing your throat”. Yes to any of them, and it advises you to “consult a doctor to determine the underlying cause.”

One small habit helps meanwhile. When you feel the tickle, take a small sip of water and swallow instead of clearing your throat. A throat clear is hard on the vocal folds, and it’s no help to an irritated throat.

© The Voice Realm

Habits clinics suggest for reflux

SingHealth’s patient page lists the lifestyle changes that doctors usually try first. The ones that matter to a voice actor are below, in their words where they’re short enough to quote.

The same page says spicy and fatty food, orange and grapefruit juice, fizzy drinks, milkshake, coffee, tea and alcohol can worsen reflux. That’s a long list, and nobody’s going to give up every item on it overnight. The easier route is to find out which ones matter for you.

Four habits to try

Smaller meals. The page says to “eat smaller, more frequent meals”, because a large meal expands the stomach and increases upward pressure.

A gap before lying down. It says, “Do not lie down for three hours after you eat.”

A higher head at night. Raising the head of the bed or using a mattress wedge can help.

No smoking. Nicotine relaxes the muscle that keeps stomach contents down.

A plain food and voice diary does that job. For two weeks, write down what you ate, when you ate it, when you went to bed and how your voice felt the next morning and mid-afternoon. Patterns show up quickly. Maybe it’s the late curry, maybe it’s the third coffee between auditions, maybe it’s nothing you’d have guessed. You can take the notebook to the doctor as well, and doctors like specifics.

© The Voice Realm

Planning your day around your stomach

This is where freelancing helps you. You often control your own schedule, and a few small decisions make a difference.

Eat your main meal at a sensible time and not at 10 pm because the last session ran late. If you know a long recording day is coming, plan the food in advance: something light and plain before you start, a decent break at lunch, and a proper dinner that leaves you hours before bed. Keep water within reach of the mic, and sit up straight rather than slumped over the script, which gives your stomach and your lungs both a little more room.

If you also warm up in the morning, it still helps. Our five-minute warm-up loosens the voice, but it can’t undo a night of reflux irritation, so don’t push through a rough morning voice by forcing it. The NIDCD’s advice is blunt: “Avoid speaking or singing when your voice is hoarse or tired.”

© The Voice Realm

Sleeping with your head raised

Bed is where reflux does a lot of its quiet work, because lying flat makes it easier for stomach contents to travel upward. The patient page says it “sometimes helps to have the head higher than the stomach” and suggests “raising the head of the bed or using a mattress wedge.”

Note what that says: the head end of the whole bed is raised, not just a pillow. A wedge is a simple thing to try, and it’s worth a go for a couple of weeks next to the food diary so you can see whether the mornings improve.

© The Voice Realm

When to see a doctor

Voice work is your income, so don’t wait for a rough patch to become a crisis. SingHealth says you should see a doctor “if your symptoms persist despite lifestyle adjustments, become more severe over time, or noticeably impact your quality of life.” It also lists signs that need attention sooner: “trouble swallowing, bloodstained saliva or vomitus, neck swelling or masses, or unexpected weight loss.”

The NIDCD points voice complaints toward an otolaryngologist, a doctor who specialises in the ears, nose and throat, “sometimes called an ENT”, who can look at your vocal folds directly. That doctor may refer you to a speech-language pathologist, who can help you improve the way you use your voice. Tell them you use your voice for a living, bring your food diary, and ask what they’re seeing before you start any medicine. The Voice Foundation findings above are a good reason to ask what the plan is for after you start it, too.

If your voice is sore because of a cold and not reflux, our guide to recording voice over with a cold covers when to record, adjust or rest, and the broader vocal health guide sits behind both.

Sources: SingHealth, “Laryngopharyngeal Reflux”; Hathway, Best, Weber, Holland and Perrine, “Reflux Experiences in Collegiate Vocal Performers”, Voice Foundation, 2025 abstracts; National Institute on Deafness and Other Communication Disorders, “Taking Care of Your Voice”.

Silent reflux can roughen a voice with no heartburn at all.
